  • For every death, a certificate of death or fetal death must be filed within 10 days of the death.
  •  
  • The death certificates must be filed by the person in charge of internment or disposal of the body, or by the person in charge of removal of the body from the registration district for disposal.
  •  
  • A funeral director is considered a legal representative or agent of the family of the deceased. Generally, a part of the services provided a family include obtaining certified copies from the local registrar.
  •  
  • This office issues full copy certifications. All certified copies are issued on special security paper and each certificate has a unique document and serial number.

  • A death certificate is a confidential document for the first 25 years after filing and may only be issued to a properly qualified applicant. (Requesting Death Certificate by Mail)

  • The proper registrar for filing is determined according to the place of death, not by place of disposition or location of the funeral home.
  •  
  • Burial transit permit (VS-116): A burial-transit permit is required to ship or transport a body out of this state or to transport a body by common carrier within this state. It is also required for cremation. The clerk will issue a permit to the funeral home by completing the permit (VS-116) and giving the permit to the funeral home representative.

  • Phone calls from funeral homes regarding corrections will be addressed to the Sr. Clerk or Supervisor.
